It’s a switch-off title, but it heralds a revolution in chemotherapy:“Metronomic chemotherapy prevents therapy-induced stromal activation and induction of tumor-initiating cells,” was the breakthrough study published online on November 23 in The Journal of Experimental Medicine.

‘Metronomic chemotherapy’means slow and steady drip-feeding of chemotherapy drugs, in contrast to the standard regime of “maximum tolerable” doses given for a few weeks, with long respite periods in between to allow the healthy parts of the patient’s body to recover from the toxic assault.

The problem with the standard all-out “war on cancer” regime is that not all cancer cells are killed, allowing the surviving cells to use the respite periods to respond in kind. The survivors create proteins which encourage the growth of fresh, highly aggressive cancer cells; these create more tumours not only at the existing tumour site but also throughout the body in a process called metastasis. And it’s metastasis that usually kills you, not the tumour.

Metronomic chemotherapy avoids all this, according to the recent JEM study on mice by UCSF researchers. A very few pioneering oncologists are beginning to test this stratagem on their patients, and the word is they’re getting good results (as yet unpublished).
But don’t wait for human trials. If you really do decide to wage war on your cancer using chemotherapy, don’t allow your doctor to use a barrage of big guns, choose guerilla infiltration instead.
